Sending Records to National Jewish Health

If you want your medical records from another healthcare provider mailed to National Jewish Health, please comply with the following:

  1. Download, complete and print this authorization form.

  2. Mail or hand deliver the form to your physician and/or hospital where services have been provided to you.


Courtesy Reports

As a courtesy, National Jewish Health will provide you and your referring physician a copy of the dictated summary from your initial visit. It is important that you have the complete name, address, phone and fax number of the referring physician upon check-in of your appointment. Your referring physician information will be entered into the National Jewish Health computer system ensuring you and your physician receive a copy. Sometimes completion of your summary may take up to 30-days, especially if specialized testing was done.  If you have not received your initial report after 30-days, you may call 303-398-1580.


Obtaining a Copy of Your Medical Records

In most circumstances, National Jewish Health is required to obtain your written consent before we are able to release a copy of your confidential health information.  We are not able to accept email requests. Download, complete and print this authorization form, making sure to include your handwritten signature.

Requests for records to be sent to a healthcare provider can be faxed to: 303-398-1211.

Note: Requests for all information can delay processing, become very costly and can sometimes be much more information than you really need. Generally, copies of the physician report and testing is all that is needed for continuing medical care.  If you need help deciding what to request please contact the HIM Department at 303-398-1580.


Payment for Copies of Records

Copies of records are provided at a charge in accordance the Colorado Standards for Hospitals and healthcare Facilities, 6 C.C.R. 1011-1, Chapter 2 Part 5.2.3.4.

Copies of records (1-10 pages) - $14

  • 11-40 pages - Additional 50 cents per page
  • 41+ pages - Additional 33 cents per page

Copies of records are provided to healthcare providers at no charge.

These charges are due at the time you receive your records in person. If you wish records to be mailed, no pre-payment is due. An invoice will be sent to you with a copy of the records.


Receiving Your Requested Records

Once the HIM department receives your authorization to release information, it will take approximately 7-10 business days for the record to be photocopied and mailed to the address you provide.  Occasionally delays occur.  We will attempt to contact you if you extra time is needed to process your request.

Note: The Health Information Management Department can only provide copies of the information we have.  We cannot interpret what the information means or discuss it with you. If you have any questions regarding the information, you will need to contact your physician.

After reviewing your records, if you feel something needs to be revised or corrected, please download, complete and print the Request Amendment to Medical Record form.
