Schedule an Evaluation or Inquire About Our Programs

If you would like to schedule an evaluation at National Jewish Health Main Health Campus , or talk to someone about our programs, please contact LUNG LINE. You can do this in several ways:

  • Online Form: Complete the Online Patient Pre-Registration Form. You will be contacted to process your request.
  • Phone: Call LUNG LINE at 1-800-222-5864 Monday through Friday, 8:00 am to 4:30 pm Mountain Time (10:00 am to 6:30 pm Eastern Time). Our specialized respiratory nurses can talk to you about our programs and how you might benefit from an evaluation. Also, the nurse will collect some medical and insurance information to help with scheduling.
  • Email Inquiry: Send LUNG LINE an e-mail at [email protected].
  • Physician Referral: If your physician would like to refer you for an evaluation at National Jewish Health, please ask him or her to contact the Physician Line.
  • Case Manager Referral: If your case manager would like to refer you for an evaluation at National Jewish Health, please ask him or her to call the Case Manager Line.

If you are traveling to National Jewish Health from outside the greater Denver metro area, plan on staying with us for one week. Find information on lodging and discounted rates here.
